关于来华留学生入学体检要求的说明
根据中国教育部的有关规定，结合实际情况，西安外国语大学对于留学生入学体检的要求如下：
一、入校留学生均须体检。
二、申请来华学习的学生，在申请系统上提交符合要求的体检材料。申请180天以内学习的学生，也可在申请系统上提交“入境后体检申请”，并在入境后两周内在指定机构体检。（陕西国际旅行卫生保健中心，地址位于西安市含光北路10号）
三、持X1签证的学生，入境后30天内须办理“居留许可”，所需材料其中一项是《境外人员体格检查记录》；此材料须由指定机构出具，申请方式有两种：1、已有体检报告材料的，由本人持报告及所有相关材料，在指定机构进行材料审核验证（审验不合格者须重新体检或补检指定体检项目）。2、没有体检报告材料的，须在指定机构体检。（陕西国际旅行卫生保健中心，地址位于西安市含光北路10号）
四、无论持有哪一类签证，如果在中国指定机构体检结果不合格，则必须按照法律规定限期离境，产生的各种相关费用和后果均由个人承担。
五、陕西国际旅行卫生保健中心体检费用标准（截至2024年10月）：全套体检费用303元；血检+X光检查188元；仅血检153元；仅X光检查35元。已有体检材料经审核后合格者，免费出具《境外人员体格检查记录》。（注意：学生拿到体检记录请先扫描留存，如申请第二份体检结果，将收取相应费用。）
六、 外宾境外体检材料验证：
（1）《外国人体格检查记录表》所体现的内容应由中文或英文填写，签发日期，应清晰明了，此表有效期从签发之日起六个月内有效。此表须包括受检者照片（加盖医院公章）、体检结论、医生签名、医院公章（同照片处公章一致）。
（2）体检项目按《国际旅行健康检查证明书》的内容进行， 附X光片、心电图图纸或报告、实验室检测报告（至少包含HIV、梅毒检测、丙肝）、超声检查报告等原始报告或者化验单。
（3）体格检查及体检报告的签发必须由当地的公立医院完成；如在私立医院或诊所完成，体检报告应先在当地进行公正。
（4）体检报告内容如须通过中国驻外国领（使）馆的认证的，申请人应向中心工作人员提供使馆的认证证明或者认证标签。
Explanation on the Physical Examination for International Students' Enrollment
According to the relevant regulations of the Chinese Ministry of Education and based on the actual situation, Xi'an International Studies University has the following requirements for the entrance examination of international students:
1.All international students entering the university are required to undergo a physical examination.
2.Students applying to study in China must submit the required medical examination materials on the application system. Students who apply to study within 180 days can also submit a "post entry medical examination application" on the application system and undergo a medical examination at a designated institution within two weeks of entry. (Shaanxi International Travel Health Care Center, located at No. 10 Hanguang North Road, Xi'an City)
3.Students holding X1 visas must apply for a "residence permit" within 30 days of entry, and one of the required documents is the "Physical Examination Record of Overseas Personnel". This material must be issued by the designated institution, and there are two application methods: (1)If there is already a physical examination report material, students could present the report and all relevant materials to the designated institution for material review and verification (those who fail the verification must undergo a new physical examination or supplement the designated physical examination items). (2)If there is no physical examination report material, a physical examination must be conducted at the designated institution.
4.Regardless of the type of visa held, if the physical examination result at a designated institution in China is not qualified, the individual must leave the country within the prescribed time limit according to the law, and all related expenses and consequences incurred shall be borne by the individual.
5.The medical examination fee standard of Shaanxi International Travel Health Care Center (as of October 2024): the full set of medical examination fee is 303 yuan; Blood test and X-ray examination costs 188 yuan; Blood test costs 153 yuan; X-ray examination costs 35 yuan. For those who have already passed the physical examination materials review, a "Record of Physical Examination for Overseas Personnel" will be issued free of charge. (Note: Students are requested to scan and retain their medical examination records. If they apply for a second copy of the examination results, corresponding fees will be charged.)
6.Verification of overseas physical examination materials for foreigners:
(1) The contents of the Foreigner Physical Examination Record Form shall be filled in Chinese or English, and the date of issuance shall be clear. The form is valid for six months from the date of issuance. This form must include a photo of the examinee (stamped with the hospital seal), the medical examination conclusion, the doctor's signature, and the hospital seal (consistent with the seal in the photo).
(2) The physical examination items shall be conducted according to the contents of the International Travel Health Examination Certificate, accompanied by original reports or laboratory test reports such as X-rays, electrocardiogram drawings or reports, laboratory test reports (at least including HIV, syphilis testing, hepatitis C), ultrasound examination reports, etc.
(3) The issuance of physical examination and examination reports must be completed by local public hospitals; If completed in a private hospital or clinic, the medical examination report should be notarized locally first.
(4) If the content of the medical examination report needs to be certified by the Chinese Embassy in another country, the applicant should provide the embassy's certification certificate or certification label to the center staff.
